Child's Full Name and Child's Preferred Name (Required)
Please include the Child's Full Name as you would like it to appear on the printed certificate and also include the Child's preferred name as you would like it to appear on the big screen during service. (Example: Alyssa Madison Combs and Ally Combs)
Relationship and Names of who is dedicating the child
For printed certificate, example: Grandparents Don and Marie Shackelford
Photo needed too!
Please email Angie.therefuge@gmail.com or TEXT 918-200-2479 with "CHILD'S NAME: Refuge child dedication" and include a photo - this will be used on the printed certificate and on the big screen during the service. You can send a family photo or a photo of just the child and you can send a few photos and we will pick the one that fits the best.
